Zgłoszenie

Zgodnie z art.13 i 14 Rozporządzenia Parlamentu Europejskiego i Rady (UE) 2016/679 z dnia 27 kwietnia 2016 r. w sprawie ochrony osób fizycznych w związku z przetwarzaniem danych osobowych i w sprawie swobodnego przepływu takich danych oraz uchylenia dyrektywy 95/46/WE, Dziennik Urzędowy UE, L 1 19/1 z 4 maja 2016 informuję, że:
1. Administratorem Pani/Pana danych osobowych jest Polskie Towarzystwo Bibliodramy. Kontakt: sekretariat@bibliodrama.org
2. Pani/Pana dane osobowe są przetwarzane w celu rejestracji udziału w Europejskim Kongresie Bibliodramy w dniach 4-8 sierpnia 2020.
3. Ma Pani/Pan prawo do dostępu do treści swoich danych oraz ich poprawiania, usunięcia, ograniczenia przetwarzania, prawo do przenoszenia danych, prawo do wniesienia sprzeciwu wobec ich przetwarzania. Ponadto przysługuje Pani/Panu prawo do złożenia skargi do organu nadzorczego, gdy uznane zostanie, iż przetwarzanie danych osobowych narusza przepisy ogólnego rozporządzenia o ochronie danych osobowych z dnia 27 kwietnia 2016 roku.
4. Pani/Pana dane nie będą przekazywane innym podmiotom.
5. Pani/Pana dane osobowe będą przechowywane przez okres niezbędny do realizacji Kongresu. Po zakończeniu Kongresu zostaną usunięte.
6. Podanie przez Panią/Pana swoich danych osobowych jest dobrowolne, jednak ich nie podanie uniemożliwi Pani/Panu udział w Kongresie.

In accordance with Articles 13 and 14 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC, I hereby inform you that:
1. Your personal data is administered by the Polish Bibliodrama Association. Contact: sekretariat@bibliodrama.org.
2. Your personal data is processed in order to register for the European Bibliodrama Congress (4-8th August 2020).
3. You have the right to access your data and to correct, delete, limit the processing, transfer and object to the processing of your data. Furthermore, you have the right to lodge a complaint with the supervisory authority if you believe that the processing of your personal data violates the provisions of the General Data Protection Regulation of 27 April 2016.
4. Your data will not be transferred to other entities.
5. Your personal data will be stored for the period necessary to implement the Congress. After the end of the Congress they will be deleted.
6. Providing your personal data is voluntary, but not giving them will prevent you from participating in the Congress.

I have read the above information and accept the terms and conditions provided in it.

Gemäß den Artikeln 13 und 14 der Verordnung (EU) 2016/679 des Europäischen Parlaments und des Rates vom 27. April 2016 zum Schutz natürlicher Personen bei der Verarbeitung personenbezogener Daten und zum freien Datenverkehr und zur Aufhebung der Richtlinie 95/46/EG teile ich Ihnen mit, dass:
1. Ihre persönlichen Daten werden von der Polnischen Bibliodrama-Vereinigung verwaltet. Ansprechpartner: sekretariat@bibliodrama.org.
2. Ihre persönlichen Daten werden verarbeitet, um sich für den European Bibliodrama Kongress vom 4. bis 8. August 2020 anzumelden.
3. Sie haben das Recht, auf Ihre Daten zuzugreifen und diese zu berichtigen, zu löschen, die Verarbeitung zu beschränken, zu übertragen und der Verarbeitung Ihrer Daten zu widersprechen. Darüber hinaus haben Sie das Recht, bei der Kontrollstelle eine Beschwerde einzureichen, wenn Sie der Meinung sind, dass die Verarbeitung Ihrer personenbezogenen Daten gegen die Bestimmungen der Allgemeinen Datenschutzverordnung vom 27. April 2016 verstößt.
4. Ihre Daten werden nicht an andere Unternehmen weitergegeben.
5. Ihre persönlichen Daten werden für den zur Durchführung des Kongresses erforderlichen Zeitraum gespeichert. Nach dem Ende des Kongresses werden diese gelöscht.
6. Die Angabe Ihrer persönlichen Daten ist freiwillig, aber die Nichtangabe dieser Daten hindert Sie an der Teilnahme am Kongress.
